PLEASE READ THE FOLLOWING INSTRUCTIONS / GUIDELINES CAREFULLY BEFORE APPLYING FOR REISSUANCE OF PASSPORT OR ANY OTHER RELATED MISC
PASSPORT SERVICE.

APPLICATION/DOCUMENT SUBMISSION TIME IS FROM 0930 TO 1230 HRS ON ALL WORKING DAYS.

IT IS AN OFFENCE, TO HOLD MORE THAN ONE VALID PASSPORT AT A TIME AND FURNISH FALSE INFORMATION IN THE APPLICATION, UNDER THE PASSPORT ACT 1967. PASSPORT FACILITIES WOULD BE DENIED ON GROUNDS OF SUPPRESSION OF MATERIAL INFORMATION, SUBMISSION OF INCORRECT PARTICULARS, WILLFUL DAMAGE/LOSS OF PASSPORT OR FOR UNAUTHORIZED CHANGE/TAMPERING. THE PASSPORT CAN BE IMPOUNDED OR REVOKED FOR VIOLATION OF ONE OR MORE OF THE PROVISIONS UNDER THE PASSPORT ACT.

INDIAN NATIONALS ARE STRONGLY ADVISED TO APPLY ATLEAST SIX MONTHS BEFORE THE EXPIRY OF VALIDITY OF THE PASSPORT. THIS WILL HELP THEM IN AVOIDING SITUATIONS AS SOME COUNTRIES REQUIRE A MINIMUM SIX MONTHS' PASSPORT VALIDITY FOR ISSUING VISAS.  A PASSPORT EXPIRED FOR MORE THAN 6 MONTHS SHOULD BE SUPPORTED BY A HAND WRITTEN LETTER EXPLAINING THE CIRCUMSTANCES AND THE REASONS FOR LATE RENEWAL.

AN ORDINARY PASSPORT IS VALID FOR TEN YEARS FROM THE DATE OF ISSUE. IT MAY PLEASE BE NOTED THAT THE PASSPORT ISSUING AUTHORITY, CAN ISSUE PASSPORTS EVEN FOR A SHORTER DURATION. IF THE PASSPORT IS ISSUED FOR A SHORT PERIOD FOR ANY REASON, IT CAN BE REISSUED FOR A MAXIMUM PERIOD OF TEN YEARS. MINORS ARE ISSUED PASSPORTS FOR 5 YEARS OR UP TO ATTAINING 18 YEARS OF AGE, WHICHEVER IS EARLIER.

THE APPLICANT SHOULD SUBMIT TWO RECENT CLEAR PHOTOS 51MM*51MM (2 INCH X 2 INCH) AND NOT OLDER THAN THREE MONTHS, NO UNIFORM, WITH WHITE BACKGROUND AND DARK DRESS WITH FRONTAL VIEW. EYES OPEN, EARS, FOREHEAD & CHIN TO BE COMPLETELY VISIBLE, NO SHADOWS ON THE FACE AND NECK, NO REFLECTIONS ON THE GLASSES.

ANY CHANGES/CORRECTIONS TO BE AMENDED IN THE PASSPORT SHOULD BE ACCOMPANIED WITH SUFFICIENT DOCUMENTARY EVIDENCE, REQUEST LETTER WITH PROPER EXPLANATION. ORIGINAL PASSPORT NEEDS TO BE SUBMITTED FOR ALL KIND OF SERVICES. ALL APPLICANTS SHOULD SUBMIT THE APPLICATION IN PERSON FOR IDENTIFICATION. ALL SIGNATURES IN THE APPLICATION FORM SHOULD BE IN A DARK BALLPOINT PEN OF BLACK AND SIGNED IN THE PRESENCE OF SERVICE OFFICER. THOSE WHO CANNOT AFFIX THEIR SIGNATURES LIKE INFANTS, MINORS OR ILLITERATE SHOULD PUT THEIR THUMB IMPRESSION IN THE PLACE OF SIGNATURE IN THE PRESENCE OF SERVICE OFFICER.

AS PER THE LATEST INSTRUCTIONS, POLICE VERIFICATION IS COMPULSORY BEFORE RENEWAL/ISSUANCE OF A PASSPORT IN RESPECT OF ALL PASSPORT APPLICANTS WHO HAVE NOT BEEN RESIDENT ABROAD CONTINUOUSLY FOR THE LAST 5 YEARS, AT THE TIME OF APPLYING FOR RENEWAL/ISSUANCE OF A PASSPORT.  IN ALL OTHER CASES ALSO, POLICE VERIFICATION, POST RENEWAL/ISSUANCE OF PASSPORT, IS COMPULSORY. APPLICANTS SHOULD SUBMIT DOCUMENTARY PROOF OF RESIDING ABROAD CONTINUOUSLY FOR THE LAST 5 YEARS AT THE TIME OF SUBMITTING THE PASSPORT APPLICATION.

POLICE VERIFICATION FROM INDIA FOR THE ADDRESS(ES) THE APPLICANT HAS STAYED AT EARLIER IS NOW MANDATORY FOR ALL SUCH APPLICATIONS AND THE PASSPORT WILL BE PROCESSED ONLY AFTER THE POLICE VERIFICATION REPORT FROM INDIA IS COMPLETED.

DUE TO THE POLICE VERIFICATION REPORT BEING MADE MANDATORY FOR REISSUE OF PASSPORT/POLICE CLEARANCE CERTIFICATE (PCC), INDIAN NATIONALS SEEKING THESE SERVICES SHOULD KEEP SUFFICIENT MARGIN OF TIME WHILE APPLYING. HENCEFORTH, THE NORMAL TIMELINE NOW FOR THESE SERVICES WILL BE 15 DAYS, OVER AND ABOVE THE TIME; IT TAKES FOR RECEIPT OF POLICE VERIFICATION REPORT FROM INDIA.

(i) Reissuance of Passport on Expiry/Exhaustion of pages/Misc. Services such as inclusion of Spouse name, Change of Address or any other endorsement:

Documents required

  1. Old passport in original with photocopies of first, second and last page.
  2. Alien Registration Card in original with photocopies (front & back).
  3. Two recent passport size photographs with white background. (2 inch x 2 inch) *A digital copy of the same photograph must also be submitted to ICAC.
  4. Printout of the online submitted Application Form (Click Link: https://mportal.passportindia.gov.in/mission/ to fill online)
  5. Duly filled Personal Particulars Form (Form No. 2 to download)
  6. Duly filled ‘Annexure-D’ by Parents in case of applying for Minor.
  7. Photocopy of any two documents for Identity/Address Proof along with originals for verification. (i.e. Driving License, PAN Card, Aadhaar Card, Voter ID Card, Ration Card, Electricity Bill etc.) *In case of changing of Address, the provided documents should be updated with the new address.
  8. Addition of Spouse name: Apostilled original and copy of Marriage Certificate.
  9. Misc form

(ii) Reissuance against Lost / Damaged Passports:

Documents required

  1. FIR/Police report. (In case of Lost Passport). *PASSPORT NUMBER MUST BE INCLUDED IN THE POLICE REPORT
  2. Details/Photocopy of passport. (In case of Lost Passport).
  3. Damaged passport in original with photocopies of first, second and last page.
  4. A letter explaining how the passport was lost / damaged.
  5. Original and Photocopy of Alien Registration Card (front & back).
  6. Photocopy of Visa showing entry date in South Korea. *If unavailable, please obtain an official Entry and Exit Certificate from a nearby community center. Kindly request that the certificate cover the period from your date of birth to the present.
  7. Two recent passport size photographs with white background. (2 inch x 2 inch) *A digital copy of the same photograph must also be submitted to ICAC.
  8. Printout of the online submitted Application Form. (Click Link: https://mportal.passportindia.gov.in/mission/ to fill online)
  9. Duly filled Personal Particulars Form (Form No. 2 to download)
  10. Duly filled ‘Annexure-D’ by Parents in case of applying for Minor.

(iii) Fresh Passport for Newly Born Child:

Documents required

  1. Both Parent’s passports in original with photocopies of first, second and last page.
  2. Original and Photocopy of Alien Registration Cards of both Parents (front & back).
  3. Photocopy of Both Parents’ Visas showing entry date in South Korea. *If unavailable, please obtain an official Entry and Exit Certificate from a nearby community center. Kindly request that the certificate cover the period from your date of birth to the present.
  4. Two recent passport size photographs of child with white background. (2 inch x 2 inch) *A digital copy of the same photograph must also be submitted to ICAC.
  5. Printout of the online submitted Application Form. (Click Link: https://mportal.passportindia.gov.in/mission/ to fill online)
  6. Duly filled Personal Particulars Form (Form No. 2 to download)
  7. Duly filled ‘Annexure-D’ signed by both Parents.
  8. Child’s Birth Certificate in English from Hospital. *For minor applicants whose parents hold G-1 visas, an apostilled birth certificate is required.
  9. Printout of duly filled online Birth Registration Form on MHA’s website (Link: https://indiancitizenshiponline.nic.in - Go to Section 4 and complete the form titled “Registration of Birth of a Minor Child at an Indian Consulate (Under Section 4(1) of the Citizenship Act, 1955)”.))
  10. Misc form

Note-
Applicants are required to furnish original documents along with one set of self-attested copies of the same for processing. For minor applicants, documents must be attested by both parents EITHER PARENT CARRYING APPLICATION FORM, SUPPORTING DOCUMENTS AND ORIGINAL PASSPORTS OF BOTH PARENTS CAN VISIT THE EMBASSY.

*In signature columns of the application form, Right hand thumb impression in case of Female, and Left thumb impression in case of Male is required, Both parents are required to sign on the last page of the application form.

***For single parent/ guardian or adoption/separation cases, or if one parent has not given consent or If one of the parents is presently residing in India/Abroad then Annexure 'C' signed by the parent who is accompanying the child is required. If parents are divorced then Custody Paper, Divorce Decree is required. If it is death case then Death Certificate needs to be attested by Home Department of the concerned State Government in India.

(iv) Reissuance of Passport to Minor:

Documents required

  1. Old passport in original with photocopies of first, second and last page.
  2. Parent’s Passports in original with photocopies of first, second and last page.
  3. Original and Photocopy of Alien Registration Cards (front & back) of both Parents and the Minor Applicant.
  4. Photocopy of Visas of both Parents and the Minor Applicant showing entry date in South Korea. *If unavailable, please obtain an official Entry and Exit Certificate from a nearby community center. Kindly request that the certificate cover the period from your date of birth to the present. *For minor applicants born in the Republic of Korea, this requirement applies only to the parents. 
  5. Two recent passport size photographs of child with white background. (2 inch x 2 inch) *A digital copy of the same photograph must also be submitted to ICAC.
  6. Printout of the online submitted Application Form. (Click Link: https://mportal.passportindia.gov.in/mission/ to fill online)
  7. Duly filled Personal Particulars Form (Form No. 2 to download)
  8. Duly filled ‘Annexure-D’ signed by both Parents.

Note-
Applicants are required to furnish original documents along with one set of self-attested copies of the same for processing. For minor applicants, documents must be attested by both parents. EITHER PARENT CARRYING APPLICATION FORM, SUPPORTING DOCUMENTS AND ORIGINAL PASSPORTS OF BOTH PARENTS CAN VISIT THE EMBASSY. APPLICANTS OVER 15 YEARS OF AGE ARE REQUIRED TO VISIT THE EMBASSY IN PERSON.

*In signature columns of the application form, Right hand thumb impression in case of Female, and Left thumb impression in case of Male is required, Both parents are required to sign on the last page of the application form.

(v) Passport to the Overstayed Indian nationals / G-1 card holders :

  1. *A Short Validity Passport (SVP) will be issued based on the declaration from the applicant that he/she will regularize his/her overstay in South Korea after re-issuance of the valid passport.

Documents required

  1. Self Declaration Form.
  2. Old passport in original with photocopies of first, second and last page.
  3. Original and Photocopy of Alien Registration Card (front & back).
  4. Photocopy of Visa showing entry date in South Korea. *If unavailable, please obtain an official Entry and Exit Certificate from a nearby community center. Kindly request that the certificate cover the period from your date of birth to the present.
  5. Two recent passport size photographs with white background. (2 inch x 2 inch) *A digital copy of the same photograph must also be submitted to ICAC.
  6. Printout of the online submitted Application Form (Click Link: https://mportal.passportindia.gov.in/mission/ to fill online)
  7. Duly filled Personal Particulars Form (Form No. 2 to download)
